Risk Profile ReportTM Authorization Form

Reports on Subject individuals ("Report or Reports") provided under the scope of this engagement may be considered investigative consumer reports as defined under the Fair Credit Reporting Act.  If any future Reports, or portions thereof, shall be in the form of an investigative consumer report for any reason, Client must send the Subject of the Report within three days of the date on which the Report was requested, notice that such a Report may be made, and that the Subject may make written requests regarding the nature and scope of the investigation.  For total financing in amounts below $150,000 in principal, the duration of the Report covers a period of seven years preceding the date that Client authorizes the Report.  For total financing in excess of $150,000 in principal, the period of coverage may be longer, as determined by the location(s) and date(s) at which we find the Subject individual(s) to have resided (the "jurisdictions") and the type of Report (Level 1 or Level 2) selected below by the Client.  A Level 1 Investigation covers a period of 7 to 10 years and a Level 2 Investigation covers a period of 10 to 20 years.  Check the appropriate box below.  Subjects will be searched in all identified jurisdictions, based on our standard locator procedures.
